Dear Patients,
I am retiring from dermatology and Mohs surgery on July 1, 2026.
My active patients (patients who have been seen continuously over the last three years) will need to choose a new doctor to make sure medical care continues.
You may be able to see Dr Ali Banki or his physician assistants at Banki Dermatology in Glastonbury, CT. This will depend on his availability and your care needs.
I have known Dr Banki since 2010, he is an excellent dermatologist and highly recommended.
You may choose Dr Ali Banki, his physician assistants or you may choose a different dermatologist.
Steps to take if you choose Dr Ali Banki as your new doctor
* Call the office (860-659-2779)
Steps to take if you choose a different doctor
* Decide who your new doctor will be. For help finding a new doctor you can call your insurance company or your hospitals doctor referral service.
* Fill out and mail back the Medical Records Release Form, which was recently mail to active patients
* When we receive the form, we will send your records to your new doctor
OR
* I choosing to pick up records yourself, call our office at (860-529-2500) to set up a time to pick up a copy of your records
* Connecticut law allows a small fee to copy records of $0.65 per page
Medical records information
Your medical records will be stored electronically for 7 years (required by Ct state law).
After that they will be safely destroyed.
